Gilbert's syndrome is an inherited (usually autosomal recessive) metabolic disorder that causes intermittent, isolated raised unconjugated bilirubin levels, due to defective conjugating enzymes in the liver. There is otherwise normal liver function, and no evidence of haemolysis. Gilbert’s syndrome (GS) is a condition in which you have higher than normal amounts of bilirubin in your blood. The medical name for this is ‘unconjugated hyperbilirubinemia’. It is also sometimes called familial nonhaemolytic bilirubinaemia or constitutional hepatic dysfunction. The proportion of conjugated bilirubin with respect to the total raised bilirubin can give a clue as to cause of jaundice. Note that if conjugated bilirubin is less than 20% of total then the cause of hyperbilirubinaemia is secondary to haemolysis or constitutional e.g. Gilbert's disease, Crigler-Najjar syndrome. This enzyme that makes the bilirubin water soluble is called glucuronyl transferease, known as UGT for short, and is what people with Gilbert’s Syndrome are genetically lacking. This means that the bilirubin is not ‘conjugated’ or converted so that it can easily leave the body. Mildly elevated total bilirubin with normal serum activities of liver transaminases, biliary damage markers, and red blood cell counts, however, may indicate the presence of Gilbert's syndrome (GS), a benign condition that is present in ∼5-10% of the population.
